Micro TESE

TESE is a surgical procedure who is applied to the men without sperm cells in the semen.  The lack of sperms cells in the semen might have several reasons including;

  • Obstruction in sperm tubes
  • Congenital inexistence of sperm tubes
  • Hormonal reasons
  • Genetic reasons; the genes, which provide sperm production in men, are on Y chromosome. The problems with these genes affect the sperm production.
  • Radiation; especially having radiation for the cancer treatment may sometimes eliminate the sperm production and this can be permanent.
  • Medication; especially the drugs used in cancer treatment may stop the sperm production permanently
  • Infections ( After Mumps)

How is micro TESE procedure done?

Micro TESE is a painless procedure which is applied under anesthesia. Tissue samples are taken from at least four different points of both testicles under a microscope. Those tissue samples are gone through a series of processes and live sperm cells are separated. These cells are used in the microinjection procedure. Micro TESE procedure differs from testicular biopsy.  In biopsy, a sample is taken from a single point of the testicle. However, there might be sperm production in other parts of the testicles. Therefore, the sperm cells can be obtained through micro TESE procedure from the patients who had testicular biopsy but did not have sperm cells.

The sperm cells which are obtained as a result of micro TESE are frozen and stored and only then the patient is taken into the IVF treatment program. Otherwise, if no live sperms cells are found, it would be unnecessary to start the treatment with the woman.

Freezing the sperm cells does not violate the quality of them. Additionally, it prevents the necessity of having micro TESE again when there is a need for second IVF attempt.

Things to be cautious about before micro TESE

  • You should not eat or drink anything for at least 6 hours before the operation (including water)
  • You should clean the operation area with a razor blade and have a bath.
  • You should bring a loose trousers or sport suits because pressure on the testicles will cause pain after operation
  • You will be discharged in 1-2 hours after the operation

Things to be cautious after micro TESE

  • Liquid food intake can be started within 3 hours after the operation
  • You can have a bath on the 5th day after the operation
  • Do not have sex for a week
  • You can use painkillers if you have any pain
  • Always report to your doctor if there is redness, swelling or bleeding
  • Self-melting stitches are used during micro TESE so there is no stitching removal procedure.

Risks

As a result of micro TESE, live sperm cells may not be obtained. This risk depends on the underlying cause. The chance of finding sperm in a patient with obstruction in the sperm tubes is higher than the patient with hormonal or genetic problem. After surgery, there may rarely be bleeding in the operation site.
